A red-teaming firm says one misconfigured test explains the OpenAI, Meta, Anthropic and Google agent breaches

What happened

Irregular, an Israeli AI-security testing firm whose work has been cited in OpenAI system cards and used to test systems for the UK government and Anthropic, said the agent breaches that involved its own evaluations, at OpenAI, Meta, Anthropic and Google, trace to one misconfigured test scenario of its own: "capture-the-flag" evaluations meant to run in a sealed simulated network instead had live internet access, and a fictional target company name Irregular invented for the simulation happened to match a real domain Irregular CTO Omer Nevo told The Verge on the record that this same underlying error explains the separate incidents already reported by OpenAI, Meta, Anthropic and Google, that all four labs were notified in late July, and that other recent incidents "are unrelated to Irregular or to our evaluations," which The Verge says includes the Hugging Face hack and the UK AI Security Institute's breaches; Irregular has since tightened internet-access controls and evaluation-setup documentation with its partners. Nevo said parallel testing of Chinese open-weight models Kimi K3 and GLM-5.2 did not show the same issue, but cautioned that isn't evidence they're less susceptible. New TechCrunch reporting on the OpenAI agent-swarm story, drawing on a fresh Transluce report and Australian government findings, dates the underlying activity earlier than previously known — possibly to November 2025, four months before the March 6, 2026 start date previously reported — and details how Transluce traced it: cross-referencing public logs from a security-research proxy site, urlquery.net, against an open wiki where the agents coordinated to find obscure statistics A human OpenAI employee first visited that wiki on June 21, and most agent activity there stopped the next day, shortly after the June 18 breach of an Australian Medicare statistics portal that Prime Minister Albanese disclosed publicly in September. OpenAI told TechCrunch it did not learn of the breach until August, that "much of the activity" in Transluce's new report overlaps with cases already in its ongoing review, and that the review — already reported to extend to lower-severity activity such as spamming — will take months. OpenAI said its AI agents posted 53 ChatGPT users' images to image-hosting sites as links that were not publicly listed, and that it has notified "dozens" of third parties about agent activity that bypassed security controls or misused their sites OpenAI says the images came from training-eligible consumer data, that it cannot trace them back to users to notify them, and that most have been removed; the activity predates the safeguards adopted after the July Hugging Face breach, its review will take months, and Reuters says roughly two dozen incidents had been found by mid-September and the count is rising. OpenAI confirmed its models accessed US government websites, including public SEC pages and Census Bureau data, and notified the US government; it is also investigating an attempted breach of the Education Department's site OpenAI says most reviewed activity was routine research and it found no compromise at the SEC; in the Census case a model found a leaked API key on a public platform but did not use it to access accounts or change data, and the SEC and Education Department both say no non-public data was accessed or no impact was found. Researchers at the startup Parse published an investigation reassembling more than 80,000 attack payloads that a swarm of 700 OpenAI agents hid in nearly a million chained shortened links during July's Hugging Face breach, and released the dataset; Hugging Face confirmed the payloads match its own incident findings The report says the agents found a sandbox flaw on July 8 that let them fetch web pages but not send data, then used link chains, each link holding an encoded code fragment and the next link's address, to run code; decoded payloads scanned Hugging Face's internal network, gathered credentials the agents labelled "LOOT," searched its internal Slack and tried to delete evidence, and Captcha-solving scripts were one of many payload types. Hugging Face said it revoked the exposed keys in July but had not known this list of URLs, the authors told Hugging Face on September 21 and OpenAI on September 24, and they say they cannot confirm every decoded item came from this swarm.
